Carney was one of the original members of the band Tin Huey, which is where he began his career as a professional musician. Carney has worked with Tin Huey in the 1970s and Tom Waits since the 1980s.  Carney has even recorded or performed with artists such as Dieselhed, the B-52's, Bill Laswell, Marc Ribot, Elvis Costello, Jonathan Richman, St. Vincent, Stan Ridgway, Les Claypool, Medeski Martin & Wood, Jed Davis,  Griddle, and HowellDevine, among many others. Carney was one of the core members of the Swollen Monkeys in 1981, alongside Mars Williams, who later joined the Psychedelic Furs. That same year, the band put out an album titled "Afterbirth of the Cool," which was produced by Hal Willner. In addition to launching a number of solo albums, Carney was a part of the Oranj Symphonette alongside fellow Waits veterans Joe Gore and Matt Brubeck. He was the leader of the San Francisco band Carneyball Johnson, in which he played saxophones, Turkish clarinet, trumpet, piccolo, percussion, and vocals.

Carney and the Black Keys worked together on their studio album, Attack & Release in 2008.When they were touring in support of that album, he would sometimes accompany them on stage. He worked with his nephew Patrick on the theme song for the TV show BoJack Horseman in 2014.During the fall of 2009, he embarked on a tour with They Might Be Giants.

In 2008, he collaborated with Black Francis to record and play a score for the silent movie Der Golem (1920), and in 2010 and 2011, he appeared as a guest performer with Yo La Tengo and Medeski Martin & Wood at their respective live concerts. He collaborated with Marc Ribot and Elvis Costello on a album titled The Ghost Brothers of Darkland County that was produced by T Bone Burnett but has not been launched as of yet. 

At UCLA's Royce Hall, he was a part of several of Hal Willner's productions, such as a tribute concert to Harry Smith in 2001 that featured a massive ensemble of musicians, like Philip Glass, Todd Rundgren, and David Johansen, among others.Carney contributed music to the production of two poetry recordings for Paris Records.One of them was a collaboration with the poet Robert Creeley and was titled Really!. In addition to that, he composed music for an album by Ira Cohen titled "The Stauffenberg Cycle." Ralph contributed his vocals to the album "Redoing Childhood" by Kathy Acker in 1994.In 2001, he did some tracks for CartoonNetwork.com's flash Web Premiere Toons. In 2009, his previous band, Tin Huey, launched a collection CD on Smog Veil Records that contained previously unreleased songs. He worked with David Greenberger, who makes the Duplex Planet, on a song called "Oh Pa," which was launched at the end of 2011. He began recording with and performing live with his Ralph Carney's Serious Jass Project after 2009. In July of 2011, Smog Veil Records released a new album under the title Seriously.
